A chronic condition is any condition or symptom that has long-term effects on a person, needs ongoing management, and which may impact the activities of daily life. The Managing Chronic Conditions in Irish Primary Care PSP will work with people with chronic conditions, their carers and clinicians to identify and prioritise the unanswered questions about the management of chronic illnesses in primary care.
The PSP will focus on adults ( 18+) in Ireland (ROI).
