UNCERTAINTY: How can community stroke services best be resourced and organised in all regions to provide effective home/community-based rehabilitation that meets the needs of all groups of stroke survivors such as ethnic groups, young people, stroke severities and those with multiple health conditions? Explanatory note

Many stroke survivors need professional support in the community to cope, adapt and recover in life after stroke. However, these needs are not being met as 45% of stroke survivors say they feel abandoned when they leave hospital [Ref. 2].

The resourcing and organisation of stroke services in the community are variable, which impacts the appropriateness, access to and the quality of the services. Individuals support needs also vary depending on multiple factors.

Evidence is needed to understand how to resource and organise these services, and the needs of different groups of people, in order to inform how services are delivered on a local basis.
