Original uncertainty examples Can access to heart failure nurses be increased within the UK? How do you best access a heart failure nurse and what is their role with the MDT?
 ~ Who is best to talk to patients about end of life and advanced care planning? How do we recognise when a patient is becoming palliative and who is best placed to make this decision - GP, heart failure nurse, Cardiologist ~ Can a HF patient be effectively managed by a nurse practitioner? ~ Traditionally not concentrated on in secondary care although very important for patient and relatives. HF nurses do this much better than Drs. would be useful to know what exactly patient want to know and whether the current info is sufficient, who they would like to get this info from ~ Are we doing a good enough job and do patients know who to contact/ what to do when they have a problem ~ Are outcomes for patients with severe heart failure better if they are under heart failure nurse care or GP care
